Retailers Accused of Selling Fake Supplements

GNC, Target, Walgreens and Walmart -- are accused of selling fake and potentially harmful herbal supplements and must remove them from their shelves, the New York State attorney general's office has announced.
An investigation by state authorities found that a number of top-selling store brands of herbal supplements at the four major retailers did not contain any of the herbs listed on their labels, The New York Times reported.
Instead, they contained cheap fillers like powdered vegetables and houseplants, or ingredients that could pose a threat to people with allergies, officials said.
Here are some of the specific finding from the investigation, according to The Times report: Walmart's ginkgo biloba -- marketed as a memory booster -- contained little more than powdered radish, houseplants and wheat, even though it claimed to be wheat- and gluten free. Walgreen's popular store brand of ginseng pills contains only powdered garlic and rice. Three of six herbal products at Target -- St. John's wort, ginkgo biloba, and the purported sleep aid valerian root -- had no herbs and were made of powdered rice, beans, peas and wild carrots. At GNC, herbal pills contained unlisted fillers such as powdered legumes, a class of plants that includes peanuts and soybeans, which can trigger allergic reactions.
On Monday, the state attorney general sent cease-and-desist letters to the four retailers and told them to outline the procedures they use to verify the ingredients in their herbal supplements, The Times reported.
"Mislabeling, contamination and false advertising are illegal," State Attorney General Eric Schneiderman said. "They also pose unacceptable risks to New York families -- especially those with allergies to hidden ingredients."
In response to the investigation, Walgreens said it would remove the products from its stores nationwide, Walmart said it would "take appropriate action," and GNC said it would cooperate "in all appropriate ways," but added that it stood behind the quality and purity of its store brand. Target did not respond to requests for comment, The Times reported.
Many health experts who have long warned about the quality and safety of dietary supplements welcomed the investigation. It's the first time that large retail and drugstore chains in the United States have been threatened with legal action for selling misleading herbal products, The Times reported.
"If this data is accurate, then it is an unbelievably devastating indictment of the industry," Dr. Pieter Cohen, an assistant professor at Harvard Medical School and an expert on supplement safety, said.

Take Heartburn Medicines Before Breakfast

Many people with heartburn aren't taking their acid-reducing medicine at the right time, which makes the drugs less effective and wastes money, according to new research.
Only about one-third of those buying these medications -- such as Nexium, Prevacid and Prilosec -- over-the-counter used them properly compared to just under half of those who were prescribed the drugs by their primary care doctor. Those who were given a prescription by a gastroenterologist were most likely to use the drugs as they're supposed to be used, with seven out of 10 taking the drugs properly, according to the study.
These drugs are activated once in the body, said the study's senior author, Dr. M. Michael Wolfe, a gastroenterologist and chair of the department of medicine at MetroHealth System. "In order to activate the medicine, you must eat. For that reason, you take it before breakfast. If you don't take the drug correctly, you don't do as well," Wolfe said.
Despite labels advising users to take the drugs before breakfast, people aren't following those directions, he said. Those who aren't taking the medicines properly "are wasting money, they're not feeling well and they aren't getting symptom relief," Wolfe added.
The study was published in the June issue of The American Journal of Gastroenterology.
Heartburn is a painful, burning feeling just below the breastbone, experienced at least once a month by about 44 percent of U.S. adults. About 7 percent have heartburn daily. Frequent heartburn may indicate a condition called gastroesophageal reflux disease, or GERD. Food and acid from the stomach backs up, or refluxes, into the esophagus. Reflux can damage the esophagus and cause serious issues over time.
Direct costs related to GERD, including acid-reducing medicines, top $10 billion each year in the United States, according to background information in the study.
The medications looked at in this study are a class of drugs known as proton pump inhibitors. They work by reducing the amount of stomach acid produced, according to the U.S. National Library of Medicine. Unlike antacids, such as Tums or Rolaids, proton pump inhibitors don't provide immediate relief of heartburn symptoms. It takes about 7 days of continuous use for the drugs to reach their maximum acid-suppressing potential, the study noted.
Wolfe and his colleagues surveyed 610 patients who used heartburn medicine for their GERD. Of that group, 190 got a prescription heartburn medicine from a gastroenterologist and 223 received a prescription from their primary care doctor. The other 197 bought over-the-counter heartburn medicines.
Those prescribed the medicines by their gastroenterologist did best, Wolfe noted, with 71 percent taking the medicines correctly. Only 47 percent of those who got prescriptions from their primary care doctors took them correctly. And just 39 percent of those who bought them over-the-counter used them right, the investigators found.
In a previous study, only one-third of primary care doctors told patients to take the medicines before meals, but nearly all gastroenterologists did, according to the report.
In his study, Wolfe found, the severity and frequency of symptoms were better in those who were prescribed the drug by a gastroenterologist compared to a primary care doctor.
"If you have frequent heartburn, you have a disease, GERD," Wolfe said. "And you really should see a physician and not treat yourself," he explained.
Dr. John Lipham is director of the Digestive Health Center at Keck Medicine of the University of Southern California. Lipham reviewed the findings but was not involved in the study.
"It's something we have known since these medications came out, that they work best if you take them 30 minutes or so before a meal," said Lipham.
However, he pointed out that the new study puts some data behind what experts knew from experience.
Lipham said the new study is the first, to his knowledge, to show a difference in taking the medicine correctly depending on who prescribed it.
Wolfe and Lipham both find that patients often think of the proton pump inhibitors in the same way as antacids, meant to be taken when heartburn strikes.
"But these [proton pump inhibitor] medicines don't work that way," Lipham said. "They need to be stimulated by acid and need to build up in your system. You have to take them at the correct time each day and you also need to take them every day to get the maximum effectiveness of the medications."
As to why doctors aren't all telling their patients how to use these drugs, Wolfe speculated that primary care doctors may be too busy and don't have the time to read all of the drug literature.
The bottom line is "it boils down to education," said Wolfe. Physicians and consumers need to take the time to learn about the drugs.
Ideally, Wolfe said, you should take the medicine in the morning, then ''eat something that causes your stomach to make acid, such as protein, an egg, a piece of cheese, yogurt." For those who hate breakfast, he advises drinking a glass of milk or at least a cup of coffee.

Barr adds it to muffin batter, or to make broth for cooking soup, rice, and pasta. That's "going the extra step to fortify your meals," she says.

Do You Need a Juicing Machine?

Juicers can be expensive, ranging from $50 to $400. Some more expensive juicers will break down a lot of the fruit by grinding the core, rind, and seeds, Barr says.
You may not need a juicing machine to make juice. You can use a blender for most whole fruits or vegetables to keep the fiber. Add water if it becomes too thick, Villacorta says. You’ll want to remove seeds and rinds, and some skins.
Once your juice is ready, it's best to drink it the same day you make it, for food safety. Wash your blender or juice machine thoroughly, so it's ready for your next batch.

Watch the Calories

How many calories are in your juice? That depends on what's in it.
"You could be taking up to four fruits, and now the calories start adding up. If you use vegetables to juice, the calories are a lot less. If they use mainly vegetables, add an apple or kiwi for flavor. Calories are a concern if it’s pure fruit juice," Villacorta says.
You can make your juice more balanced by adding protein. Some good sources are almond milk, Greek yogurt, flaxseed, and peanut butter.

Juicing for Weight Loss and Cleansing

Juicing might seem like a simple way to lose weight, but it can backfire.
On a juice-only diet, you may not get enough fiber or protein to make you full. You might rebel.
“If you’re doing a juicing diet, you’ll be so tempted to eat something like a cake or doughnut because you’ve restricted yourself,” Barr says.
Not getting enough protein could also mean you lose muscle mass.
The bottom line: It's too extreme, and the results aren't likely to last.
What about juicing as a way to detox or cleanse your body? “I haven’t seen any research or science paper to support that cleansing is happening from juicing,” Villacorta says.
Your liver and kidneys take care of that -- whether you're juicing or not.

Other Health Claims

As for other health claims, it's true that eating a plant-based diet is linked to lower risk of heart disease or cancer. But there hasn't been a lot of research done that's specific to juicing.
There is some research on juicing and the immune system. But any immune system benefits probably come from eating fruits and vegetables, whether it's in juice or not, Barr says.

If You're Taking Prescription Drugs

Check with your doctor before doing a lot of juicing, so you can avoid any potential problems.
For instance, large amounts of foods high in vitamin K, such as kale and spinach, may change how the blood thinner warfarin works.

If you can change the situation, go for it. If the problem is out of your control, you can manage the way you think about it. If you can notice your stress in the moment, you can choose how you respond, rather than reacting the way you have in the past.
Consider talking with a counselor so you can better understand what's going on with you and the best ways to handle it. Even a few sessions may help.

It helps to add a delay between the urge to eat and actually eating. That gives you time to check in with how you're feeling and why you want to eat.
When you get the urge to eat a cookie out of sadness or boredom, remember that you have the option to wait it out. “Saying to yourself ‘I’ll have it later’ gives the impulse time to pass,” Farrell says. Even if it doesn’t, successfully delaying the snack helps you feel more in control.
“Wear a rubber band around your wrist, and snap it whenever you reach for the jelly beans,” Farrell says. The snap is your cue to be mindful about what's about to happen.

Get Moving

When you’re tempted to snack for emotional reasons, try moving instead.
“Just walk in place for 10 minutes,” Farrell says. Even a quick burst of activity refreshes you, and moving is a proven stress-buster. You've replaced the urge to eat with something else.

Keep It Real

The truth is not all emotional eating is unhealthy. It’s normal and natural occasionally to eat to celebrate with friends or because you’re feeling blue. “It only becomes a problem when it is used frequently and even in the face of unhealthy consequences, such as medical issues,” Becker-Phelps says.

Have you heard that apple cider vinegar will help you lose weight?
The only study to test the idea in people was done in Japan. In the study, 175 obese but healthy people took either vinegar or water daily for 12 weeks. Their diets were similar. They kept food journals. At the end of the study, those who used vinegar had lost slightly more weight. On average, the vinegar group lost 1-2 pounds over the 3-month period. They gained it all back after the study was over.
The researchers suggest that vinegar may turn on certain genes involved in breaking down fats.
The effect is probably very subtle, says Chicago dietitian Debbie Davis, RD. “It may have some benefits in terms of weight loss and weight management, but it is definitely not a quick fix."
If you want to lose weight, you’ll still need to exercise and practice portion control.

Apple Cider Vinegar and Blood Sugar

While apple cider vinegar probably won’t make you skinny, it does appear to help with diabetes and blood sugar control.
Carol Johnston, PhD, directs Arizona State University’s nutrition program. She has been studying apple cider vinegar for more than 10 years and believes its effects on blood sugar are similar to certain medications.
“Apple cider vinegar’s anti-glycemic effect is very well documented,” Johnston says.
She explains that the vinegar blocks some of the digestion of starch. “It doesn’t block the starch 100%, but it definitely prevents at least some of that starch from being digested and raising your blood sugar,” Johnston says.

Not every expert feels as confident about apple cider vinegar’s power.
"Trying to use vinegar to treat diabetes is like trying to bail out a flooded basement with a teaspoon," says Michael Dansinger, MD, director of Tufts University’s diabetes lifestyle coaching program.
He advises patients to focus instead on their overall diets -- a strategy backed by a lot more research, he says.
If you have gastroparesis, a common problem with diabetes that slows stomach emptying, be careful. Early research shows apple cider vinegar may make this problem worse.
"I’m concerned that drinking vinegar, even diluted in water, increases acid in your system, which puts a strain on your kidneys and bones,” Dansinger says.
If you have diabetes and want to try apple cider vinegar, let your doctor know, and keep an eye on your blood sugar levels.
Johnston stresses that if you are on medication for diabetes, you shouldn’t stop taking it and substitute vinegar. If you're thinking about using it to help manage your blood sugar, talk to your doctor first.

Apple Cider Vinegar and Digestion

If you drink apple cider vinegar with a starchy meal, then the starches you don't digest will feed the good bacteria in your gut, Johnston says.
Davis recommends using unfiltered apple cider vinegar, “the cloudy kind, where you can see a blob in the bottle.”
That blob is known as “the mother,” and it’s full of probiotics and other beneficial bacteria. “This kind of vinegar can support immune function and, for some people, even help with constipation,” Davis says.

Tips on Taking Apple Cider Vinegar

Don't drink it straight. It’s so acidic that it could harm your tooth enamel and your esophagus.
Don't use a lot. “Dilute 1 to 2 tablespoons in a big glass of water, and sip it along with your meals one or two times a day,” Johnston says.
